Please review the following important policies before booking your appointment.

I am no longer accepting new or discharged clients for Massage Therapy.

If it has been 12 months or longer since your last appointment, your file is considered inactive/discharged . Online booking is not available; please contact me directly to discuss options for returning to care.

Appointments at 4:00 PM or later are not available for online booking . These times are in high demand and are held by regular clients. Please contact me directly to discuss availability.

If you have never created an account, you will need to register before booking.

Pre-authorization is REQUIRED .

Insurance cards presented at the time of service will NOT be accepted.

A valid Credit Card on file is required for Direct Billing. All expired cards must be updated to maintain eligibility.

All new clients, and those who have not booked in 12+ months, must complete their own health history and consent forms before receiving treatment.

Only the person receiving the treatment can fill out and sign these forms, not a spouse, partner, friend, or family member.

Late, incomplete, or missing forms WILL result in shortened treatment time, inability to set up and submit insurance claims, or canceling/rescheduling at the client’s expense.
