▪️ To complete booking an appointment, you will be asked to create a profile on this platform. Please keep track of your log in information.
▪️ Please make sure to click “Intake Forms” in your client profile to ensure all necessary forms have been completed (Consent & Confidentiality must be signed) prior to booking and attending your first appointment.
▪️ $200/50 min individual session. We do not direct bill. You will need to submit your receipts directly to your insurance provider.
▪️ We understand life happens, but if you cannot keep the scheduled appointment, you MUST cancel or reschedule the appointment 24 hours in advance of the scheduled appointment time. If you do not show/cancel less than 24 hours in advance, you will be responsible for paying the full session fee. Future sessions will not be booked until payment is received.
Communication: I kindly ask that you limit the use of email strictly for essential administrative purposes only. This includes scheduling appointments, Jane assistance, billing/fee arrangements, or quick updates if you’re running late to an appointment. I typically respond within 48-72 hours and don’t respond to communication over the weekend. If you’ve found other services or will be taking more than a 3-month break, please let me know so I can close your file and take you off my client/email list
