For returning patients who are pregnant and have seen our chiropractors for more than 12 visits.

For patients who have never had chiropractic care at our office. (Age 19-64)

For patients who are under 3 years of age.

For patients who have never received chiropractic care in our office and who are 3 to 17 years of age.

For immediate family members, including dependent children under 19 years of age, who have not previously received chiropractic care at our clinic. Minimum of 3 and maximum of 6 family members.

For people who are pregnant and who have never had chiropractic care at our office.

For patients who have never had chiropractic care at our office and who is a student or senior (Age 65yrs+).

For patients who have not been in our office for more than 1 year.

For regular patients who are students, seniors, or who have had more than 12 chiropractic visits at our clinic.

Regular patients (Age 19-64)

For patients aged 0–2 who have previously received chiropractic care at our clinic.

For patients aged 3 - 17 who have previously received chiropractic care at our clinic.

For families who have not received chiropractic care at our clinic in over one year but less than two years. Minimum of 3 and maximum of 6 family members.

For patients requiring X-ray imaging and/or X-ray follow-up.

For patients requiring X-ray imaging and/or X-ray follow-up.

For patients who have sustained a workplace injury. An active WorkSafe claim number is required.

For patients with a workplace injury beyond the initial 8-week period. An Extension Report will be submitted to WorkSafe, and services may be billed directly once approved.

For patients who have been injured at work within the 8 week period. Please provide us with your WorkSafe claim number to process billing for this visit.
