INITIAL INTAKE FORMS : When you book your first appointment for chiropractic or massage therapy, you will be emailed an initial intake form to complete prior to your initial appointment. These forms include questions about your concerns, your health history, whether you have extended health care that we may be able to direct bill and office policy consents. It’s easy to forget these forms and put them aside, however the information is required by your health care practitioner. Intake forms MUST be completed prior to your visit. Any intake form that has not been completed prior to 24 hours of your appointment will receive an email reminder to do so. Additionally, you may receive a call from our friendly front desk staff the day of your appointment if it is still not completed.
About the clinic
DIRECT BILLING : We offer direct billing to most major insurance companies, including Canada Life, Green Shield Canada, Manulife, Medavie Blue Cross, Sun Life, TELUS Health, and many of those who allow third party billing to pay the practitioner/clinic. “My family and I have been coming here for almost 10 years. The entire experience from the moment you step in, to the moment you leave. You feel welcome, comfort and happy. The chiropractors and massage therapists are the absolute best there is! The office manager and office assistants are beyond welcoming and are always greeting everyone with a smile. We've been coming for chiropractic adjustments and massages for a long time. I've always left happy. We've been bringing our son since he was 9 weeks old. Doesn't matter what your body needs. “Highly recommend Absolutely Chiropractic. My husband has been seeing Dr. Becky for years to work through sports injuries and he recommended I see her after our car accident. Dr. Becky was not only amazing at treating me throughout that recovery process, but she was instrumental in assisting me with all the MVA paperwork and ensured I had all the documentation needed for the insurance company to fulfill my MVA claim. I could not have navigated that process without her. Dr. Becky also treated me throughout my pregnancy and postpartum, and now our toddler see's Dr. Richelle who is incredible with children. We could not recommend Dr. Becky and Dr. Richelle more.
