Online Forms : All New Patients must complete both our Health Intake and Consent Form prior to your appointment to maximize your treatment time. The forms can be found in your email upon completing your booking. There will be TWO seperate emails reflecting the TWO seperate forms .

ICBC Claims : Some practitioners are able to direct bill to ICBC for accidents that are approved and occurred within 12 weeks of accident date. Claims must be pre-approved by our office before your first appointment.

DIRECT BILLING : We are able to direct bill to majority of insurance providers such as Pacific Blue Cross, Greenshield and extended health insurance companies partnered with Telus-Eclaims (Canada Life, Manulife, Chambers of Commerce, Group Health, etc.). If we are unable to get direct confirmation of the payment after we process your claim, you may be responsible for the full cost of your appointment and have to submit the claim yourself to your insurance. We will provide you with a receipt once payment is rendered.

CANCELLATION POLICY : You are required to cancel an appointment at least 24 hours before your scheduled appointment as per our 24 hour cancellation policy . Failure to do so will limit our ability to fill your scheduled appoitment with someone else needing treatment and on the cancellation list, and will result in the full charge of the treatment cost fees billed directly to you. (Unfortunely we cannot bill these type of non-treatment/missed appointments to your insurance company).
