Upon entering, turn right immediately, and take the elevator to the 4th floor. Please walk to room 415. If you need help entering the clinic: “watch this video” * Please note that every Sunday and/or Holidays the elevator will be locked. Please call our clinic to notify that you are here, and wait outside the office tower next to HSBC on the ground floor along Kingsway. One of our team members will come down and bring you to our clinic. Parking: The parking entrance, aka “The Crystal,” is located on Kingsborough Street, in between Willingdon and McKay Ave. There is 4 levels of underground parking (P1, P2, P3, P4). Please park in any stall, except the ones labelled reserved parking. The first 3 hours of parking is free. Intake Form: Please fill out the intake and direct billing forms before your appointment. iCBC: We currently are unable to submit iCBC. Please pay in full and submit yourself.
