Welcome to Crosstown Rehab’s booking page. Note that we are located inside Crosstown Family Health Team on the second floor. An elevator is available for those who require it. If you cannot find an appointment that works for you, please contact us at 647-956-0477 or info@crosstownrehab.ca to inquire or add yourself to the waitlist. We will do our best to accommodate same or next day appointments. If you are looking to book a Rapid Access Clinic for Low Back Pain appointment, please contact our office directly and ask for Dr. Julian Velez. Note that our fax number has changed to 647-956-0477 (same as our phone number).
We accept debit, Visa, Mastercard, AMEX, and cash, and offer direct billing for most insurance companies. We also bill directly for motor vehicle accident (MVA) and WSIB claims; however, you must call us to book MVA or WSIB appointments.
24 HOUR CANCELLATION POLICY: We ask that you give us 24 business hours notice for any cancellations or changes to your appointment. If your appointment is on a Monday, please let us know the Friday before that you need to cancel. Patients who provide less than 24 hours notice, or miss their appointment, may be charged a cancellation fee of 50% for the first late cancellation/no show, and 100% for the second late cancellation/no show. If a patient consistently cancels late or misses appointments, they must pay their balance before booking their next appointment.

“I got referred to him by my family physician for my son having some issues with his knee. He made us feel extremely comfortable, spoke to my 10 year ar directly instead of asking me questions to get direct answers from him as he is the patient. Listened to every concern he had even if it seemed silly to me. He honestly is an awesome Dr and I highly recommend everyone to go to him. He is highly patient and if he can work with kids as well as he did, he must be even better with adults. I too will be now seeing him for myself and some issues I have and hope that everyone who is looking for a great chiropractor tries him out and sees how great he really is. In the past I have seen a chirpractor and all I got was a quick lemme crack everything in your body and not really assess the issues as to why they are happening and how to try exercises to fix then from reoccurring.
