We offer direct billing for most insurance companies. Your insurance plan might require that you get a doctor’s referral for physical therapy, which you need to bring on your first appointment. You also have the option of emailing it to us along with any additional reports like MRIs, X-Rays, etc. Also, please take into consideration we cannot guarantee direct billing for each and every patient. It all depends on the type of benefit plan you have with your insurance company.

NEW PATIENTS
Digital patient intake forms will be emailed to you once your new patient appointment has been scheduled. Please fill out and SIGN the forms PRIOR to your first appointment . We know your time is valuable. We want to maximize the amount of time you spend with the physiotherapist during your first visit rather than spend time filling out paperwork.
Please wear comfortable clothes. All assessments are one on one with your physiotherapist. During that hour, your physiotherapist will assess your condition and discuss patient oriented goals that will direct your treatment.

“Hussein is always welcoming, friendly and explains things in details. He’s very kind and careful how he carries out his treatment and always checks in with you while he’s doing it to ensure that you’re not uncomfortable or in pain. Hussein has treated me for rotator cuff issues, residual nerve pain post surgery, vertigo, and Achilles tendon pain. All of which I have regained mobility and been able to move on relatively pain-free now. His assistant, Rachel, is very friendly and carries out her physical therapy exercises with you very efficiently. “Without a doubt the best medical experience I’ve ever had. I had been suffering for over a week with vertigo and was searching online to find someone to help me. After reading the reviews, I called Brant Lakeshore Physio. Hussein answered the phone himself, and agreed to see me at 7pm the same day, even though they were supposed to close at 7pm. My husband joined me for the appointment and was blown away by Hussein‘s knowledge and care. He took his time explaining everything to me and ruling out a number of issues before diagnosing the problem. He showed us a video of exactly what was happening to me so we could fully understand it. The manoeuvres he performed fixed my vertigo 90% after the first visit. I returned two days later for a second visit and I’m as good as new. The girls are the front desk were super friendly and chatty as well. I’ll make the 40 minute drive to his office from my house for any issue in the future.
