Direct billing is a courtesy I can provide, it is your responsibility to know your insurance coverage through your provider, and the patient is responsible for the remaining balance of their visit.
If you are unable to make your appointment please provide 24 hours notice.
Cancellations made under 24 hours will be charged 50% the appointment fee.
Cancellations made under 4 hours will be charged the full appointment fee.
No-Shows will be billed the full fee of the appointment.
Cancellation charges can NOT be billed to extended health plans.
