It will guide you on what to expect and how to prepare for your visit.</p><hr><p><strong>Location</strong></p><ul><li>In-Person Appointments: I see clients one-on-one at my Brooklyn office by appointment only.</li><li>Home Visits: Available upon request. Please email me for details.</li><li>Online Sessions: Sacred Energy Activation and Purpose Discovery Program sessions are conducted via Zoom.</li></ul><hr><p><strong>Required Forms</strong></p><p>To ensure your privacy and a seamless session:</p><ul><li>All clients must sign the HIPAA Confidentiality and Release Form.</li><li>First-time acupuncture clients must also complete the Patient Consent and Health History Form online before the appointment.</li></ul><hr><p><strong>Session Flow</strong></p><p>To dedicate the full session to your care, all payments must be made in full prior to your treatment.</p><hr><p><strong>Timeliness</strong></p><ul><li>Arrive at your scheduled time or 5 minutes early to relax before the session.</li><li>If you arrive late, your session will still end at the originally reserved time to accommodate subsequent clients.</li></ul><hr><p><strong>Rates and Payment</strong></p><ul><li>Acupuncture Rates:<ul><li>New Patient Consultation and Treatment (90 minutes): $275</li><li>Follow-Up Treatment (90 minutes): $250</li><li>Follow-Up Treatment (60 minutes): $225</li></ul></li><li>Virtual Sessions:<ul><li>One hour virtual sessions: $225</li></ul></li><li>Payment Details:<ul><li>Payment is required in full prior to your appointment.</li><li>Credit cards are required to secure your booking and will be charged unless another payment method is provided.</li><li>Cash and checks are also accepted. Please note a $35 fee applies to returned checks.</li></ul></li></ul><hr><p><strong>Cancellation Policy</strong></p><p>I value your time and strive to provide high-quality care for all clients.</p><ul><li>To avoid charges, cancellations or rescheduling must be made by 9 AM on the business day prior to your appointment.</li><li>Late cancellations or no-shows will incur the full appointment fee, charged to the credit card on file.</li></ul><hr><p><strong>Health Insurance</strong></p><p>A superbill can be provided for submission to your health insurance provider for potential reimbursement.</p><hr><p><strong>How to Prepare for Your Session</strong></p><ul><li>Clothing: Wear loose, comfortable clothes that can be rolled above your knees and elbows.</li><li>Food: Avoid heavy meals one hour before and after your treatment.
